 DJFL 2021 Equipment Pick-up for Tackle and MTF players

 

1st Equipment pickup: Monday June 7, 6:30-8:30pm (raindate Tues June 8) 

 

2nd Equipment pickup: Monday June 14, 6:30-8:30pm (raindate Tues June 15) *only if unable to come to first pickup date

 

Darien High School Stadium (on the black top between the track and the high school)

 

 

Please refer to the proper checklist depending on how you have registered:

1.     Registered and Paid online, go to Checklist #1

2.     Registered but did not pay online, go to Checklist #2

3.     Have not registered, There will be no In-Person Registration at Equipment Pickup. Registration is Online and in advance only.

 

Checklist #1: Registered and Paid Online

  • Player must be present at Equipment Pick-up
  • DJFL Doctor Certification Form signed by your Doctor (note that we will not accept all the various physical forms, school forms or camp forms—MUST BE DJFL FORM and must have the Doctors STAMP as well as signature). ALL other forms were completed online--THANK YOU.
  • Equipment deposit check for $400 made out to DJFL and post dated 12-1-21 (One check per player)

 

Checklist #2: Registered BUT NOT Paid Online

  • Player must be present at Equipment Pick-up
  • DJFL Doctor Certification Form signed by your Doctor (note that we will not accept all the various physical forms, school forms or camp forms—MUST BE DJFL FORM and must have the Doctors STAMP as well as signature) ALL other forms were completed online--THANK YOU.
  • Equipment deposit check for $400 made out to DJFL and post dated 12-1-21 (One check per player)
